How the Nashville Connection Program works.....

Families of children with serious emotional disturbances (SED) are assigned a Family services Coordinator (FSC).
The FSC will work with the family to include everyone who is presently involved with the family to develop a Child and Family TEAM.
The FSC will do a strengths-based assessment, which will help the TEAM develop a comprehensive and coordinated plan.
The plan will gain the knowledge needed to make positive choices for their child.
Positive choices will help keep children as home, in their community, and at school; not just for today but for the long term.

Family members

Teachers

Case Managers

Juvenile Justice

Faith Community

Coaches

Mental Health Professionals

Scout Leaders

Department of Children's Services

Anyone who has a relationship with the child or family
