Nashville Connection Eligibility Criteria

Children Who:

1. Are under age 18
2. Have *SED (Diagnosis and Functional Impairment)
3. Require the services from more than one agency
4. Have a birth or foster home in Davidson County
5. Are at imminent risk of placement in state custody or a hospitalization/residential placement
   OR
   Are in an out of home placement or state custody and could return home if support was there.
6. Has a birth or foster family in Davidson county with a primary caregiver who is in agreement to participation in a system of care with the intended goal of the child remaining in the community.

*Serious Emotional Disturbance per the mental health definition (Valid DSM IV diagnosis and GAF below 51)
